The use of robotic-assisted surgery is expected to increase, particularly by gynecologist oncologists.
Robotic surgery has increased in popularity in recent years, especially in the area of benign hysterectomy. Hysterectomy is a common operation performed in gynecology. Nearly 600,000 hysterectomies are performed in the United States every year, out of which, almost 84% are for non-cancerous lesions. Over the years, the da Vinci robotic system has gained extensive acceptance in gynecology due to enhanced visualization and excellent dexterity as against conventional laparoscopic techniques. Approval of da Vinci surgical system for use in gynecologic surgery led to significant growth in robotic procedures. Hysterectomy has been the largest and most rapidly growing procedure for robotic surgery on the da Vinci system. FDA approval of the da Vinci robot for gynecologic surgery has also led to an increase in minimally invasive abdominal surgery for pelvic organ prolapse. In 2018, Intuitive Surgical commanded a little over than 80% of the robotic-assisted surgery system market share, with over $3.5bn in sales of its da Vinci systems, instruments and accessories.
With respect to particular indications for robotic-assisted laparoscopic hysterectomy (RLH), it exhibits potential benefit over conventional laparoscopy in overweight women and in instances of large uteri. RLH offers advantages such as decreased blood loss, less postoperative pain, and shorter hospital stays as compared to vaginal hysterectomy, laparoscopic-assisted vaginal hysterectomy, and total laparoscopic hysterectomy. According to analyst reports, surgeons (especially those inexperienced in conventional laparoscopic procedures) in the USA may prefer robotic procedures as robotics is easier to learn.
On the basis of the success that laparoscopy had already attained, surgeons created and adopted robotic myomectomy. Because it is a suture-intensive surgery, the robotic approach has grown in favour. Robotic arms make suturing straightforward and easy, allowing surgeons with limited or no laparoscopic training in suturing technique to execute the treatment in a minimally invasive manner. However, drawbacks of robotic-assisted myomectomy include a limited field of vision and the inability to apply torque, which makes the removal of big myomas difficult. In cases of myomas greater than 10 cm, beyond the pelvis, a hybrid method (traditional laparoscopy with robotic aid) is recommended.
Through conventional laparoscopy, surgery for endometriosis is technically challenging. Robotic assistance may facilitate the procedure through a detailed and magnified 3D surgical view and enable better quality surgical dissection in complex cases.
While robotic-assisted gynaecologic surgery has witnessed increased adoption, high costs associated with such systems may hinder market growth.
