The major epidemiological limitation is the absence of a uniform definition and surveillance denominator for Salmonella outbreaks across the 8MM. The United States combines active population-based surveillance with WGS-supported multistate investigations, while European countries differ in notification requirements, case definitions and coverage. France uses voluntary notification, Italy has important sentinel components, Spain has estimated population coverage of approximately 82% for its salmonellosis surveillance system, and India largely reports outbreaks through syndromic surveillance while laboratory networks provide pathogen-specific information. Consequently, individual country outbreak counts should not be interpreted as directly comparable measures of national disease burden.
Management of uncomplicated NTS infection is primarily supportive, with oral or intravenous rehydration, correction of electrolyte abnormalities and monitoring for complications. Antimicrobial treatment is generally reserved for patients with invasive disease, severe illness or increased risk of complications. Infants, older adults, immunocompromised patients and people with bacteremia require particular clinical attention. Antimicrobial susceptibility testing should guide therapy when resistant or invasive Salmonella infection is suspected. Outbreak management requires rapid laboratory confirmation, case finding, epidemiological interviewing, traceback of food and animal exposures, environmental sampling, product recall where indicated and genomic comparison of isolates.
