Several advantages of employing cementless knee prostheses have been suggested, these include reduced operating room time, ease of revision, preservation of bone stock, and removal of complications linked to cemented fixation such as third body wear and retained loose fragments. Such advantages are expected to drive the global cementless total knee arthroplasty market. Osteolysis patterns are also different, (depending on mode of fixation). Osteolysis linked with cementless implants exhibits an expansible pattern in the metaphyseal bone that does not normally interfere with component fixation. A key indicator for using cementless total knee arthroplasty is good bone quality with high metabolic activity, to promote biological fixation. Younger age (less than 65 years old) and sufficient bone stock are distinctive indications.
While cementless implants are more expensive than cemented ones (as bioactive surfaces require high technology to produce), cementless total knee arthroplasty saves a significant amount of time. Decreased operative time is an important factor to consider, and reduces costs associated with intervention. Also, advancements in and use of new technologies, such as hydroxyapatite coatings or trabecular and porous metal, have allowed these implants to offer greater reliability. These factors are expected to drive the global cementless total knee arthroplasty market. Furter, demand for total knee arthroplasty has been continually increasing; with the current age of individuals with osteoarthritis decreasing, cemented fixation may not offer satisfactory long-term outcomes due to failure of fixation.
