Biliary stenting describes the insertion of stents (tubes made of plastic or metal) to release obstruction in the biliary tree or to treat biliary leaks. It may be employed to relieve obstruction for benign and malignant biliary tract conditions, and also palliative treatment of increased malignancies of the biliary tract. When there is a blockage in the bile duct, fluids like bile (bilirubin) are not able to flow into the duodenum to aid digestion. As a result, bile may build up in the liver and show symptoms like jaundice, abdominal pain and nausea. A biliary stent is inserted to decrease or eliminate blockage. Once placed in the obstructed area, the stent is designed to expand and open the channel so that fluids may continue to flow to the intestine.
While biliary stenting is extensively used to relieve malignant obstruction or treat benign biliary diseases, recent and important changes have been made to self-expandable metal stents (SEMSs), which come in multiple designs to suit different treatment techniques. Such developments are expected to drive the global biliary stent market. Plastic stents differ in calibre, length, material and configuration. Plastic stents are available in straight (angled, curved) or pigtail shapes. Pigtail stents are used when there is risk of stent migration and may have a single pigtail or one at each end (double pigtail). Amsterdam-type straight plastic stent is a commonly used stent. These contain holes at the edge of the stent to facilitate flow of bile. The distal end is tapered to assist insertion, with a flap at both ends to avoid migration. Stents with new designs continue to be developed. For example, the wing stent, which dispenses with the central lumen in favour of a plastic tube with a ribbed surface in a star shape. This maintains the duct calibre and functions as a wick for flow of juice along its surface, preventing occlusion of side branch openings.
Bare metal stents suffer from tissue ingrowth and cannot be removed endoscopically. A polymer coating may significantly influence stent outcomes. Partially covered stents cover a major portion of the lumen but leave substantial portions of the stent ends uncovered. Fully covered stents with polymer coating cover almost the whole stent, sparing a very small length at the ends. Both covering designs offer the advantage of deceased tissue ingrowth and ease of endoscopic removal.
