Urinary incontinence has been on the rise in Europe. Urinary incontinence increases with age from 14% in people aged 65–69 years to 45% in individuals aged 85 years or above. It is suggested that fifteen percent to twenty five percent of hospitalised patients and five percent of patients in elderly homes have a urinary catheter. In fact, the indwelling urethral catheter is an essential tool for many hospitalised patients. For a long time, urinary catheters have been employed to drain the bladder when it fails to empty. Clean intermittent self-catheterization may be a good alternative for people with impaired bladder function. Several people may need an indwelling catheter; for short and the long term, Foley catheter has been used. The burden of urinary retention and incontinence (in terms of social, medical and economic resources) is significant in Europe; cases have also been aggravated by the use of existing catheters, such as Foley catheter. Therefore, efforts are being put in to develop safer and easier catheter systems.
It has been reported that indwelling catheter use has increased significantly over the last two decades; clean intermittent catheter use has risen as well. The key driver behind the increase in use of indwelling and clean intermittent catheters has been an increase in incontinence care in older patients and the adaption of preferred clean intermittent catheter use in professional guidelines; these factors are likely to boost the European urinary catheters market.
In a normal urinary tract, regular flushing of the urethra as the bladder empties assists in impeding the ascending infection of the tract by bacteria that generally are present in the periurethral skin. Any bacteria that do migrate into the bladder are washed out during micturition. Further, the bladder is lined by urothelial cells coated with a glycosaminoglycan mucin, which offers a surface resistant to bacterial adherence which initiates invasion of the urothelium. This triggers microbial-sensing proteins in the superficial umbrella cells, activating host defences with a flow of cellular and molecular effectors to eliminate bacteria.
Individuals experiencing impaired bladder function (retention or incontinence) need a safe and reliable system to gather and contain the urine (for short- or long-term). In such individuals that find it feasible, clean intermittent self-catheterization is a good alternative. It mimics normal bladder function and allows the bladder to fill and periodically empty entirely, minimizing infection risk. Indwelling catheter may have to be used for patients that find clean intermittent self-catheterization not feasible. Condom catheter may be used for male patients with urinary incontinence (for short- or long-term use). However, this method carries several disadvantages and may not be suitable for every individual.
Efforts are being put in to develop catheters with antiseptic and antimicrobial coatings to decrease the incidence of catheter-related urinary tract infections.
